How to Request Flexible Work Arrangements

Approvals & Documentation

All flexible work arrangements must be requested, approved, and documented using a Flexible Work Arrangement Agreement Form, located in Banner 9 Employee Self-Service and follow procedures implemented by the Office of Human Resources.  

All proposed flexible work arrangements require approval by the employee’s immediate supervisor, the employee’s second-level supervisor, and review by the Office of Human Resources for compliance.

Employees

Procedures for Requesting a Flexible Work Arrangement

  1. Review the Policy and Terms & Conditions of Agreement, then decide if a flexible work arrangment is right for you, your position and your department.
  2. Discuss the request with your supervisor.
  3. Complete Request for Flexible Work Arrangement Form in Banner 9 Self-Service (Employees Tab-->Flexible Work Arrangement Agreement-->Request a Flexible Work Arrangement Agreement).  Click to view an Example Form
  4. Await supevisor & 2nd level supervisor approval. HR will review your submission for compliance (HR does not approve or deny requests)  
  5. Once all approvals are obtained and HR has reviewed for compliance, you may begin your flexible work arrangement.  *All flexible work arrangements are position-specific. Approved arrangements do not transfer with an employee who changes positions within the University and arrangements applying to a specific hire into a position do not automatically transfer to subsequent hires for the same position.  A new request will need to be initiated by the employee.

Supervisors & 2nd Level

Procedures for Approving Employee Arrangements

  1. Discuss the options with your employee(s).  The Employee is responsible for reviewing the Policy and Terms & Conditions of Agreement and filling out the Flexible Work Arrangement Request Form.
  2. You will receive an email notification once an approval is in your queue.  You may review the request in Banner 9 Self-Service (Employees Tab-->Flexible Work Arrangement Agreement-->Take Action on a Flexible Work Arrangement Agreement). 
  3. Click on the down arrow beside the employee's name.  You may Approve, Decline, or Return for Update.
  4. If you are the immediate supervisor, the request will route to the 2nd level supervisor.  If you are the 2nd level supervisor, the request will route to HR for Compliance Review.  HR will review the submission for compliance (HR does not approve or deny requests).
  5. Once all approvals are obtained and HR has reviewed for compliance, the agreement will be recorded in Banner for reporting purposes.    *All flexible work arrangements are position-specific. Approved arrangements do not transfer with an employee who changes positions within the University and arrangements applying to a specific hire into a position do not automatically transfer to subsequent hires for the same position.  A new request will need to be initiated by the employee.

ADA Accommodations

Flexible work arrangements are determined and administered separately from reasonable workplace accommodations under the Americans with Disabilities Act (ADA). Flexible work arrangements are not intended to substitute for the interactive reasonable accommodations process required under the ADA or to meet the University’s ADA obligations. Requests for ADA workplace accommodations must be made through the University’s Office of Disability Resources.
